Welcome to the team!
Now that you are on board, there are a few things we need from you to ensure we can use your participation.
There are two forms we will need you to fill out and sign to be a part of this project.
1. The general image release form: This is the standard image release form for film that allows the production and distributor to use your image in their movie and marketing. This states that you are aware that we are filming you and using your image and likeness for the film.
2. The Behind the Screams release form: Screamify is simultaneously filming a behind-the-scenes documentary on the making of this film that will feature the cast and crew and the process of indie filmmaking. This form also allows the use of your image/likeness to be used in the documentary and marketing.
3. Code of Conduct: General code of conduct that outlines general expectations of behavior on set.
4. W-9 form: A W-9, officially called the Request for Taxpayer Identification Number and Certification, is used by businesses and other entities to collect a person’s or business’s taxpayer identification number (TIN), name, and address. This information allows the requester to report payments to the IRS, such as income paid to independent contractors, freelancers, or other non-employee workers, as well as interest, dividends, or certain financial transactions. As a contractor, we will not withhold taxes, and it is your responsibility to report and pay your taxes.
All forms need to be filled out. If you do not wish to be a part of the documentary, we will be unable to use you in the film. There is no efficient way for us to have you participate in one and not the other.

During filming, we will need several background extras in multiple scenes throughout production. Extras will be featured in various scenes portraying coffee shop patrons, police officers, restaurant patrons, Chiropractic patients, YouTube Fitness Models, AA group members, light rail riders, and general Seattle residents.
While filming takes place, Screamify will also be on location filming ourproduction for their original docu-series, "Behind the Screams",
*You must agree to appear in both the film and documentary. If you do not agree to both, we are unable to cast you in the film.
